Engaging Staff in Individualizing Care
The Pioneer Network Starter Toolkit is a product of the Pioneer Network's National Learning Collaborative on Using the MDS as an Engine for High Quality Individualized Care. It provides information on how 49 nursing homes were able to accelerate their improvement by using four key organizational practices: consistent assignment, shift huddles, involving CNAs in care planning and Quality Improvement (QI) closest to the resident.
