The TMF Quality Innovation Network works with health care industry experts who are interested in making a lasting change as part of a community coalition in their area in Arkansas, Mississippi, Nebraska, Puerto Rico, Texas and the U.S. Virgin Islands. As a member of a community coalition, health care leaders work together to address the challenges associated with the following topic areas, as defined by the Centers for Medicare & Medicaid Services.

- Improve Chronic Disease Self-Management
- Increase Vaccinations
- Improve Quality of Care Transitions

As a member of a community coalition, health care providers have the opportunity to:

- Receive free access to social determinants of health data and metric dashboards to support data-driven quality improvement in their community
- Work with you to improve your patients' ability to manage their chronic diseases with a focus on patients who have heart disease, diabetes and kidney disease.
- Attend free, live, web-based educational forums and affinity groups facilitated by TMF subject matter experts to build an “all share and all learn” atmosphere
