The TMF Quality Innovation Network invites health care providers to join the Quality Improvement Initiative (QII) Network to receive more information about tools and resources to improve processes and performance.

The Centers for Medicare & Medicaid Services has authorized the TMF Quality Innovation Network to provide technical assistance to health care providers and practitioners in all care settings in Arkansas, Mississippi, Nebraska, Texas and the U.S. territory of Puerto Rico and the U.S. Virgin Islands to improve the quality of care for Medicare beneficiaries. This technical assistance may include provider and practitioner education and assistance with development of a quality improvement initiative (QII). A QII is any formal activity designed to support quality improvement efforts through the use of proven methodologies.

QII Template and Resources

The TMF Quality Innovation Network has created a template to use when you are requested to perform a QII. This template will assist you with performing your root cause analysis and determining what appropriate interventions to implement and monitor. Download this QII plan template in English (PDF) or this QII plan template in Spanish for Puerto Rico (PDF) and, once completed, submit it to the TMF Quality Innovation Network by fax at 1-800-725-6245 or email it to QIINetwork@tmf.org. A TMF Quality Innovation Network quality improvement specialist will contact you to confirm receipt of the template and begin to work with you.

QII Referrals

The opportunity for a QII can come from a variety of sources, including a referral from the Beneficiary and Family Centered Care (BFCC) Quality Improvement Organizations (QIOs). Below is contact information for each BFCC QIO for a specific state in the TMF Quality Innovation Network Region.
