Good afternoon everybody welcome to today's call. Um, we are going to be presenting today on navigating your way through the system. My name is Bea Young, I am one of the healthcare quality consultants with TMF health quality Institute, and I am welcoming you to the call today. I'm going to go ahead and share with you a little bit about how to navigate through our platform this morning afternoon for some of you. And, um that way, we can ensure that you have all of the tools at your disposal to be able to interact with us in this interesting conversation that we're gonna have this afternoon. So, there's a couple of ways that you guys can go ahead and ask questions during the course of the presentation. There is a Q and a feature, and it should be located at the bottom of the panel. When you expand that by pressing the little Chevron next to Q, and a, you will see the option pop up, and you want to make sure that your questions are addressed to all of the panelists. So that we can make sure to obtain your question and get you a response for your question. The next way that you can connect with us is if you want to go ahead and raise your hand so we can go ahead and unmute your line. Um, your hand is located next to your name. You should be able to see the little hand feature next to your name. And that will signal us that you have a question that you would like to ask and we will go ahead and unmute your line. So that you can ask a question. And and we can have that interaction with you and then also you want to go ahead and watch the chat feature for links. That will be placed on there. I'm gonna go ahead in a moment here and drop in the link. That will allow you to download a PDF version of the presentation that we will be using today. And through that PDF version, you will be able to make your notes. And also we will go ahead and send that follow up with the recording of this conversation that we're gonna have today. So, with that said, I want to go ahead and introduce our speaker this afternoon. Uh, YOLANDA is a healthcare quality improvement specialists with TMS, health quality Institute. And before joining worked as a long term care administrator, and was the director of a quality improvement and infection control in acute care hospitals for more than 35 years she was certified in both health care quality for acute care facilities and long term care. Quality improvement obtained her lean Sigma, her 6 Sigma link green and black belts certifications from Purdue University and she holds a bachelor's of science and nursing from the University of Puerto Rico a long term care administrator degree from San Marcos University in addition to a master's in healthcare administration and gerontology. Ms. fellas is also a TMF. Subject matter expert for long term care facilities and hospitals where she assists end users with technical assistants and provides training to ensure quality reporting to. So, it is my pleasure to welcome YOLANDA. And thank you, Yolanda for Um, posting this information for our hospitals on the H, quick contract, and we appreciate all of your guidance and support, um, going to go ahead and pass you the control. So you can advance the slides and. Welcome. Okay. Thank you so much and good afternoon. It is. My pleasure to be here today to help you learn how to navigate an image. Today we have a great agenda, and I hope you learn and enjoy the presentation. We will be discussing the difference between and and the security access management services, Sam's registration and how to complete that process. We will also want to explain how to update in facility administrator. Hospital administrator and has a position, you know, with the person who. Manages the images an account, um, and it is called the in facility administrator, so just wanted to mention that and then we will also list the steps to confirm rice to. In the benefits of confirming in addition we will do some demonstration will do a demo to show you how to complete the annual survey, how to map your facility for reporting purposes how to create reporting plans. And how to manage the alerts that you will be getting from an HSM. So, let's get started. In each person you hear, the question is what is in Sam's. In HSN is the is the CDC national healthcare safety network they are the national surveillance system for infections in HSN. Um, right now it's working with a lot of different type of health care systems, including hospitals, ambulatory surgery centers, dialysis centers. Long term care, acute care organizations, the tax and long term care facilities like nursing homes and assistant living. The role of during the facility enrollment is actually to link your email to your facility so you can initiate initiate the process of getting access to the portal. I also wanted to mention that has to help desk support system, you know. That, um, they have technical support and it can be found on the website. You can also email them at unfortunately, and they just send us not have a phone number that you can call them. Everything has to be done through email. On the other side, we have the secure access management services. They are a federal information technology system. They are a 3rd party partner with the CDC, and their function is to grant and users access to the images import. For reporting purposes, they do have a phone number that is listed here on this line. It's 177 601. 29 or 1, and they can also be reached by email on Sam's help at CDC. The reason why, um. There are 2 different entities is because 1 does actually, and it just and actually, what they do is they manage. The access to the portal from you as a user on the other side grants, you the grid card to access the portal through the Sam's. Um. Link so, here, you see, you will have to profiles. That's the reason. Why you have 2 profiles um, you have 1 for and you have 1 for Sam's profile. Like, you can see here on the left side. It does have like, my user ID, which is a, and then like, the facility name. What you will find on that profile is actually your profile. To to access a facility in a specific and then you can also find there the rights that you have to access the portal for any. On the other side, the right side you can see here the Sam's profile. The Sam's profile is located on the left hand side of the CMS menu. Once you log into any just say. And on the expense profile, you will find your Sam's credentials, your email associated with the account, your name. Your home address that you register that you used to register with Sams and then your security questions. Why you need to, um, why you need both profiles. Because 1 gives you access to the facility and it just in database, which is the profile and then the other 1 gives you access to the portal, which is the Sams profile. You have emails your email in HSN shouldn't always match your email on Sams and that that's 1 of the things that many users. Sometimes they. Changed their emails and Sams and forget to change it on and then they lose the access to the facility. In order for you to, um, obtain access to a, you need to, um, be added as a user. There are 2 ways that you can be added as a user. 1 is that. If if there is someone else at your facility that has access to your, um, to, they can add you as a user. Um, using the option of user. The function of that user on the homepage, what they need to do is they need to, um. Use the user ID, right? A user ID for you and that usually is your 1st initial last name and then they need to complete, uh, those areas that have the right asterisks. So, it will be your 1st name your last name, the phone number in your email address. Some users, um, is important, you know, for you to enter. This information correctly, so that way your information is linked correctly to. Once you complete this form, then you have to scroll down to the bottom and click say. And once you save that form, you will be coming back to this, this. Right and it writes a section. And this is for you to grant access and your rights to that person that you're adding as a user. Many times I bias users to. Gifts, uh, everyone, the administrator in all rights users. Rights, and the reason is because that way you can have access to the user tab to add more users as well as the analysis tabs. There is a whole process for you to obtain access to 1 thing that you need to to do after the rice are assigned and save the user will receive an email from to acknowledge some rules of behavior from an HSN and then to document a date of training. For today's purposes, you know, because you are attending to this, this training, if you do not have, um. Access right now to you can use today's stays as a training date because the training that is actually required is the process training. If you need to document, um, once once you. Complete that acknowledgment of the rules of behavior, and you document the training date, and you submit that information to HSN within 2 to 4 hours. Usually you should receive an email from Sam's and that email will say, uh, Sam's no reply invite to register. That is actually an invitation for you to register with. So, as you see, you are already registered with any Jason, because you were added as a user now your registering with Sams, once you, once you get that email. It will have, um, it does have a user ID. Username, which is your email address that you register with Sam with. And then it will have a weird temporary password. My advice is always to every user, make sure that you highlight that weird, temporary password and right. Click and copy it because if you try to type it in into the Sims portal many times, because it does have your characters on it, it doesn't. Um, take the password if you try to, um, enter it multiple times, then your account will be locked. And then your password, temporary password, it won't work anymore. So that's why I always advised everybody to make sure that they. You know, right. Click, highlight it and copy it. And then they can go into, let's say. Go into and to that link, that's above their username. Then some acknowledgment of rules of behavior for Samsung will open. Once they accept that they can then go ahead and complete the registration form. Once they complete the registration form, they will be able to create a password. And then some security questions. There's a couple of things that are important here. One, the is always sent to the user's home address. So it's really important to make sure that your home address is accurate in that. It matches your IDs. The other thing, um, is that if you. If you go ahead and create your account. You have to create your password your security questions, make sure that you take a picture of them with the security questions. Once you complete them because if you forget those security questions, the only way to reset them is to terminate your Sam's account. You'll have to start all over again, this process and I believe me, nobody wants to go through this process twice because this is a long process. So it is really important for you to take those those, um. Additional steps just to make sure that you have that information. Then, the 3rd step is that you will have to, once you complete the security questions, then you submit that information you can go, you can close that browser and go back to your email and you will have there another email from Sam's. And it will say, now Sam's no reply identity verification request. And it's important for you to print that email because the application for the same script card is actually in the bottom of that email. Many users missed the window of opportunity to complete the process because they think that they're waiting for another application to come on as an additional email. Please make sure that you print that email. Right now, at this time, because because of the notary requirement on that application is waved. So you don't have to complete the notary section. You will be asked to complete, um. The identification verification application form, which is the application for your card. When they're asking you to complete that they will ask you to sign it dated and put your birthday on it. And then to. Um, you know, to get 2 IDs, 1 has to have a picture and 1 doesn't really have to have a picture of the secondary ID. The primary ID can be your driver license and the secondary ID can be, um, part of what they have on the list. Are like the birth certificate, the passport, the voter cards ID, um, or you can also use your your license or your nursing license, or you can use um. In nursing homes, many administrators half a license. So they used their administrator license. Um. They'll have to just documented on the notary section. You have to put, like, the license number and the expiration date. If you do not have your license with you, your nursing license, you can go to your state board of nursing and print it out out of there. Then you'll have to take those 2 IDs. Together with your application and make copies of it. And then you will be able to scan them as as a Pdf file to your computer once you save it on your computer, then you will, um, go back to that email that you got from Samson know reply. Identity verification request. It does have a link for you to upload the documents to Sam's. Please don't email them because if you email them to Sam's, what's it going to happen is that they will destroy them. Because remember, this is information, and we don't want your information on any email so, make sure that you upload them, you know, using the SMS portal because that is a secure site. Um, once you upload those documents, then. Sams will pull them from that. Um, download that. You do. And they will process them when they process your application. Initially they put us in applicant status and then once they approve you, they will go ahead and mail you the same script card. It usually takes 7 to 10 business days after, um, after they approve you to receive the SMS. Like I said, it goes to your to your home address so make sure that, um, 1 of the important things that you need to do is to confirm your your address. So this is the section where you can come from your address once you register and I say this, because sometimes people, um. You know, give these emails and they don't. They don't, um, complete the process right away. So. And they might have, you know, something going on in their lives and they probably move from 1 place to another. And it's really important to make sure that your address is accurate in Sam's every time. Because that's where your car will go. You know, they'll go, it will be mailed to that address. That's there. So, if you have moved. Make sure that you can come here to the same profile. You will log it into and then you go into this, modify my identity data. Section and here's where you can see here Here's my profile. You can see my name my email. My home address, and then towards the bottom is the security questions, make sure that this home address that's listed in your profile on Sam's is accurate. Let's say, for example, many users. Tell me YOLANDA. I don't have, um. You inaccurate driver license, because I moved like, last month um. What can I do in HSN? In Sam's accept the, um. A utility bill, so you can send a utility bill. The has your accurate, um, address. You need to send your driver license because it has your picture, but you can also send that utility bill to prove that that is your current address. And they will accept that they will take that and they will process your application. So, your identity, you know, many of you might might be concerned about. Why do I need to prove my identity is the 2nd background check? No, it's not a background check. The only thing they really want they really want to do is to prove that you are the person you say you are, because you are asking for access to, um, to facility data. So they want to make sure that you have the right to access that information. Like I say here on the notary, um, is being Wave 2 so you don't have to get a notary. To sign your application, make sure that when you, um. Receive you'll be in the lookout for receiving your cart. It actually comes in this white. General generic white envelope, and it doesn't have anything that's really going to, like, take your attention, like, coming from the CDC and stuff like that. Um, it was just say it's coming from the Department of health and that's that's what it says. But, um 1 thing that I want to make. Sure. Is that make sure that you don't throw it away? Thinking that it's just junk mail, make sure that you check and you, if you get a card in the white envelope, and you're waiting for a subsequent card, that that is actually your card before you throw it away. If you misplace your grit card, you can call Sam's. Or email Sams, and they will send you a replacement card. If, um, if you have submitted your documents and you have been waiting for your cart, you know, for more than 60 days. Then your application has H out. And then you'll need a new application. Um, the way to obtain a new 1 is to need to email, and they will send you a new 1 Via email, if you, um. If you have Sam's credit card, you travel from place to place, you can take a picture of it. You can take a picture of your gift card and then that way you have it with you. Everywhere you go. Rather than just carrying your card, because you may lose it. And I say this, because I go to different nursing homes and. Sometimes I, I'm afraid to leave you mine behind so I have it on my phone. Um, so, now, talking about what is the, the is the is the person who actually enrolls the hospital. That person can also be, you know, is the person who actually manages the users and the user's rights. However, if you have rights to you have administrator rights to even though you're not the, you still can add users and manage the rights can add, edit, and delete facility data. That is the only person that can delete data. From a facility, they have the authority to nominate groups. And share data, they can also serve multi, you know, they can also have different roles because they can be the, the contact primary contact persons for any, just for the facility. Danny, just NFA is actually the administrator of the account. It is not the hospital administrator. Many times. I have users where facilities would they say, well, the administrators, the 1 who wants to be and that's okay you know, if he wants to apply for rights and have access to inches and that's okay but he doesn't have to be because he's not it doesn't have to be administrator. Of the facility itself it can be is the administrator of the account of the. Many times, you know, you have in facility administrator that. Um, is listed for your facility and it's no longer working at your facility in that case. You want to make sure that you request a change. This, uh, this change is requested online. On the CDC that, um, website. And what is important here is that you want to make sure, you know, that you complete this information as accurate as you can. If you don't have the information available, you know, you don't know who that person who was the prior, and then you need to email and then. And give them the facility name and the number, and they will reply back to you with that information. Just make sure that you tell them you need that information you need the name and the email address. Because you need that information to be able to change to a new and Once you complete this form, then you will get an email once you are approved. Um, that you are now, the new. But getting 1 thing that is important to mention, is that getting in the role does not actually sign you up on sales. So if you. Don't have access at your facility. There's nobody there to add you as the user you need to complete this change so you can gain access to your facility but then you need to, um, email in to ask them for the invite to register in the proofing email. And they will communicate with Sam's in a request out for you. The other way to change the role is that you can do it on the site. If that person is still working as a facility, it's going to be leaving soon. Then you can ask them to go into the facility tab. And and do facility contact information. Here they will find their name listed on the facility administrator. Section, and then they can click here on the right side and and, um. And I've reassigned that role, so somebody who is a user at the facility. That is the easiest and fastest ways to do this, but not all the time. It works because sometimes the person leaves and you don't know who that person was or is not there. To reassign the role for you some key points on a role. Um, it is really important to keep it update, you know, make sure that that person is still working at your facility is working for, within HSN because remember, you know, that is the only person who can delete data. You don't want somebody who's no longer working at your facility to have the ability to go to your facility and log in and delete your data. The only, um. The is the only person that can reassign that role. So if I'm for example, if I am the person, I, for your facility. I am the only 1 that can go in to and reassign that role to somebody else. Is there any questions right now with. An access is there anything on the queue that. That you want to share with me, and just remind everybody, if you want to come off of mute and ask a question, you want to go ahead and find your name and raise your hand push the little button that has the hand icon and we can go ahead and take you off of mute, or you can submit your questions through the Q and a feature on the account. Okay, so yeah, the other question, um, if a hospital. Point of contact has a Sam squid card for a long term care facility. Do they have to reapply for a good card or a Sam's card? Or their hospital? No, they don't. They, the same square card is the same. You you only need 1 smb's, gray cart. The only thing you need to do is you need to make sure that you're added to both, um. The long term care component, um, or the skilled nursing facility, you know, as the user with the email is attached to your card and then. In that, that it also matches the 1 that you have in the hospital that way you have access to both. Because I know that there are hospitals that have skilled nursing facilities, and they're asking, you know, if they need to have 2 different accounts, you need to have 2 different accounts with because the, the long term care component for skilled nursing facilities is different than the 1 for the patient safety component, so you can use. You need to be added to both facilities to both sections to both, um, facilities near the hospital, and the skilled nursing with the same email and you can use the same Sam speaker. You don't need to have to thank you. Okay, a lot of the questions that many users have regarding, um, the Sam's record, and the access is because. You know, sometimes they apply for the grid card, and it doesn't come in. Um, and they, they think that they just you'll have to sit down and just wait, but if you have submitted your application, and it has been more than a week that you have not received any feedback on it or, at least 1 to 2 weeks. You you don't see any, any emails coming back to you or any notification you have not. You know, gotten anything from Sam's saying that you have been approved for your same square card. Please call Sam's. They will, they will when you call them many times, they will approve you right there. And then, you know, it's just make sure that you just don't let it go because remember, the application is has it has time sensitive. It's only good for 60 days. So, if you let go those 60 days, then you will lose that window of opportunity. You have to, into get another application. So, make sure that you. You know, that you follow up on YOLANDA, what if, uh, they have an account that have been locked out of either 1 of those accounts as a new user is it the same process to request. If they usually, when accounts get locked, they get locked for 2 hours. They should be able to get in. Again, however, you know, this, this last week I have a user that she, she was telling me this is a VP he has access to different places, but, you know, on this facility, she, she cannot go in and we check a couple of things to make. Sure, that her, you know. Profile was active, and it was, but what we found out was that when she went into the user rights, and it does say on on the user, if it's active, is that no so she was inactivated there. So, once we activated her back again, she was, she was able to get in. So, it's it's really important and I'll show it in the in the demo about where you go there to change that from a note to a yes, if you, if you are listed there as inactive. Okay, okay. And before we move on to the to the next section, we do have Brianna that has a question that she would like to ask. Let me go ahead. And, um, take Brianna off off mute so we can have her ask a question. Brianna. Brianna, you're off of mute if you want to ask, we can come back. Yeah if you want to continue. Okay no problem. Okay. So the next part, you know, that is important here is to go for rights. The conferring rights, the confirm right. Means that you're sharing data with your partners and agencies using an interesting group function facilities, join and and provide access to data requesting the group by group within the interest and applications you can join, um, different groups. You know, sometimes the hospitals have their own group if they, if they are. If they're part of a corporation, they might have a group, and you can share, you know, your data with them. But you have to join that group in order to be able for them to see your data. 1 thing that is really important is, like, you know, who can confer these rights, and it can be administrate somebody with administrator rights or the. How do you confirm? I'll show you that on the demo in a little bit, but I also wanted to make sure that you are aware of the benefits of sharing rights. If you share rice with the TMF group, which is our, the group that we have. Here is the user ID, the username, which is like, the number is a 39 is a user ID group. Number is a 3903andthen the password is and it has to be all caps, uh, for it to take the password 1 thing. That is important is because once you come for rise to TMF, your quality consultant can run reports. On your facility data and view your data, you know, this allows them to be able to assist you with questions as well as you look at your data to see if there's any quality data quality issues or any concerns about that are reporting. So, it is really so, it's to the benefit of the facility as well as to yourself because then we can look at your data and alert you if there's anything that. That can, um, can cause any issues with data quality. In order to do that, you will see, you will have to group click on the group tab here on the, and then you will enter the group ID. Like I said, it was 3903andthen you will put the group, uh, password once you put the group password, then. You will see this display of conferring rights and this is just like a preset raise. Basically confirm rights to groups. They can only view your data. They will not be able to alter your data, or do anything to your data at all. It's only a view option. Um, and if you confer the rights and you accept the new. The rights then you will see, um. On the membership that you have confirmed to, you will see the group listed there. In this facility and specific, you know, I printed this with the purpose of showing to you this, um, here, this is a Texas facility so it does have Texas Department of health and Human Services, um, data use agreement group. Listed here, and this is an automatic right? You know, the Department of health at every state has the rights automatic agreement with the CDC to be able to have rights to see your data. That is why they can, you know, as soon as you enter your data. They, they're able to see it. Now, moving on to the annuals and The service must be completed by the by March. 1st. Every year when I asked a lot of the, what I asked a lot of people to do is that you can go ahead if you have done like, the prior year, you can go into the survey tab and. And click on the prior, you know, click find and then do find again, it will give you the list of all the service that has been completed and you can click on the last 1 print that 1 update it. And then you can come into and enter the data for the for the upcoming here. The service become available on January every year and they are available until March. 1st. Every year, if you do not complete the survey. Before March, 1st, you will not be able to create a reporting plan for the month. So it's really important to make sure that you complete that. Um. For patients, safety components, you know, there's different type of surveys, you know, they have, um, they're surveys for hospitals so there are specific questions the hospitals Gant there's different ones for a long term care, and other type of services. So, in the patient. There is also a patient safety annual survey for inpatient rehab as well. When completing this annual survey. My recommendation is near completed on paper firms. You can also print the current 1 if you have, you don't have any prior surveys available. You can print a copy and then do it on paper before you do it on the computer. And the reason is because it does not save partial information. So, let's say you completed the question. Some of the questions that you don't know, the rest is still going to save that information that you entered on this you complete the whole survey in 1 city. So that's why it's important for you to do it. 1st and paper, and then, you know, try to work with your team. Try to work with your team, so you can get accurate information because this information is used to risk, adjust your data. So it's really important that that information is accurate. The new section that I have here is mapping your facility. Once you have enrolled in in HSN before you can start reporting, you need to map your facility and mapping your facility is actually setting up the locations, you know, um, identify your locations the different locations of patient services, uh, patient care in your facility. And this, um, the important thing about doing this is because that is the way you can. Assign events eventually, if you have events that happen at your facility that you're reporting, you can assign them to the different locations where they happen. And it helps you when you do the analysis reports to look at your patterns and trends if you have it by location. So, part of those steps that you need to follow is that you need to define the accuity level of the location. And that's where that 80% rule comes in. When you met on location, you have to map it by the type of acuity, using the 80% rule and that any 80% rule. What it means is that 80% of your patients are of a certain type. Either pediatrics, orthopedic, surgical search, so it depends on what they are, but. It has to be 80% of your patients so you can determine what is the designated type of location you want to assign. If you have a mixed, let's say, for example, you got 50% of your patients are surgical and 50% are. Let me see our medical, then you want to make sure that you do the Met search 1 because it gives you the, the mix of both. And once you create the locations. In HSN you will, um. Once you create these locations, you will assign them the number of best that you have on each. On each unit, and that will help you then be able to create your reporting plans. When you go to recreate your reporting plans, um, you are informing the CDC, what are the patient safety modules you're going to use for the month? And then also, this information is used by the CDC, and just when they are trying to gather the National benchmarks, they want to know what facilities are monitoring or doing surveillance on what areas it also represents your facility in, in plan survey surveillance remember the only the implant surveillance are the, the ones reported to the CDC, so you might be wanting to look at other things and you say well, I'll. Don't want to put it as an implant because that way I do it if I want to if if not, then I don't have to look at that data. But the ones that are in plan surveillance data. Are the ones sent in to the CMS reporting as well? It is important for you to identify the surveillance protocols that you want to use for your facility. So, in order to do that, you will select on the and reporting tab. You will click add. So, you can add your, you will have your the name of your facility, you will add the month in the year, and then you will select the different from the different models here the device associated module, the procedure module, the antibiotics. And the multi you can also select here, um. The patient, the healthcare process prevention measures as well. Um, so. Then you will select which categories are you wanting to look at? The other section that I wanted to touch on is, uh, managing the alerts you're going to be getting, you know, as soon as you start creating reporting plans, if you have not completed the summary data report, which is like, the monthly report to, um, you will start getting alerts. And this facility and specific, you know, it has 14 events, missing events and it has 61 missing summary, um, items. Once they click on that number. Let's say, for example, they click on the 14. it will open the list of, um. Of alerts that they have pending, and then they can take care of them. They will enter the other information that's missing. Once they save it then this will start clearing up. This is the list of all those will contact will come up. And so it you can also click on this ad summary. And when you click at summary, because this is what's missing on this 1, then it will open the window for you to enter the information. And then once you enter the information and you save it, the alert will clear up. So, I'm going to go ahead and if no, if. Anybody has the questions, you know, you can post it if not. I'm gonna go ahead and share my screen. So I can go into. And do the demo for you thank YOLANDA, as you open up. Um, the live demo. A portion of this Brianna was able to get her question into the Q and a, and her question is that they've had change and staff and have established a new administrator, um, that also has SMPS access but when they log in, they cannot access their facility to enter in new data, um, they've reached out via email several times and haven't received a response. Is there someone specifically that can help them with this, or something that they can do? Because they are a quarter behind our reporting. Due to this challenge. Yes, there's a couple of things that we would have to check. We'll have to check if they have an active, um. Sam's profile and then we'll have to check you. If they are actually added as a user to the facility. Or if we need to request a change, so, if you want to gather, you know, if, if they're willing to provide you with information, um, I can give them a call after the presentation. Thank you. Okay, so here, I wanted to bring you in here to HSN in Sam's. This is where your Sam's profile lives. So, you know, this is where you will go. Um. Are you able to see my screen with the. Yes, yes. Okay. Okay. So this is where the my profile lives, so that way, you know, if let's say, for example, you need to change your email and in please make sure you change it. 1st and then and then you can change it on. To change it on and then I'll have to go and click on and reporting. Took me. I apologize it took me out because I was out for too long. So I'm going to go ahead. This is how the grid cart looks.
