Age-Friendly Health Systems: Guide to Using the 4Ms in the Care of Older Adults
To help improve the quality of care older adults receive and reduce harm, the John A. Hartford Foundation and the Institute for Healthcare Improvement, in partnership with the American Hospital Association and the Catholic Health Association of the United States, designed Age-Friendly Health Systems to meet this challenge head on. The goal of the Age-Friendly Health Systems initiative is to rapidly spread the 4Ms Framework (What Matters, Medication, Mentation, and Mobility) to U.S. hospitals and medical practices to deliver reliable and effective care to older adults.
